Medal for heroism wa round medal with a diameter of 36mm. On the obverse, a plastic relief depicting an eagle fighting a snake. On the reverse, two olive branches tied with a waving ribbon, between which is the national emblem. One-sided pendant, depicting three Svätopluk rods, tied with a ribbon. Both the medal and the pendant are gilded.
Ribbon Bar: Made of yellow moiré silk, size 60 x 35 mm. In the middle of the ribbon there is an 8 mm wide white stripe, in the middle of which there is another, green, 2 mm wide. On the ribbon there is a 20 or 32 mm wide label with the inscription For Merit.
